  • Have I used credit to get more alcohol or drugs?
  • Have I gotten drunk or high even after deciding to quit drugs or alcohol?
  • Have I dodged my responsibilities like school or work because of my alcohol or drug use?
  • Have I experienced blackouts or times I cannot remember?
  • Would my life be improved if I didn’t drink or do drugs?
